Responsibilities

• Assist with patient support questions and needs online and via phone if needed English or Spanish.
• Coordinate, and escalate, issues with Authorization for SNF admission, DOS approved, DME approval.
• Follow up and verify DOS approved By IPA and closed on the Health Insurance System
• Work closely with skilled nursing facility Staff and Case Manager to ensure tasks are properly tracked, triaged, and completed.
• Improving the Communication of IPA and SNF and verify that exist a good communication and follow up for the best care of the Patient.
• Advocate for the patient and look for opportunities to improve the care experience.
